This Day in History — Phineas Gage Survives an Iron Rod Through His Brain

On this day in 1848

On September 13, 1848, a tamping iron blasted through Phineas Gage's skull — he sat up and walked to a cart — and the injury that should have killed him changed neuroscience.

The Off-Key Bard stands beside a railroad construction site in Vermont. An explosion erupts. A heavy iron rod rockets through a man's skull — and everyone nearby has every reason to assume they have just witnessed his death.

"Phineas Gage didn't just survive. His survival would become one of the most famous cases in the history of neuroscience."

On this day in 1848, 25-year-old Phineas P. Gage, foreman of a blasting crew working on the Rutland and Burlington Railroad near Cavendish, Vermont, was tamping explosive powder into a borehole with a long iron rod when a spark ignited the charge prematurely.

The tamping iron — 43 inches long, 1.25 inches in diameter at its larger end, and weighing 13.25 pounds — was launched upward like a projectile. It entered beneath Gage's left cheekbone, passed behind his left eye, tore through the frontal portion of his brain, and exited through the top of his skull. The rod landed several yards away, bloodied.

Gage was thrown onto his back.

A few minutes later, he sat up.

He was conscious. He spoke. He was helped to an oxcart and sat upright during the ride into town, talking along the way. When Dr. Edwards H. Williams arrived and found him sitting in a chair, Gage was simultaneously vomiting and describing the accident to bystanders in a matter-of-fact tone. Dr. John Martyn Harlow — the physician who would treat Gage through his recovery and document the case for medical history — arrived shortly after and found him in the same condition.

According to accounts, Gage greeted Harlow with: "Doctor, here is business enough for you."

He was not wrong.

The Injury and the Recovery

The physical survival alone would have made Gage famous. He developed a severe infection that nearly killed him over the following weeks; his family reportedly prepared for his death. Harlow continued treating him and Gage slowly pulled through. He was back in Lebanon, New Hampshire, ten weeks after the accident.

The extent of the damage was extraordinary. A 2012 reconstruction by Van Horn and colleagues, using high-resolution CT scans of Gage's preserved skull combined with diffusion imaging data from healthy subjects to model his white matter, estimated that the rod destroyed approximately 11 percent of the prefrontal cortex and damaged nearly 40 percent of the white matter connections — the neural highways linking different brain regions — while leaving the areas responsible for speech, memory, and motor function largely intact. This helps explain why Gage could walk, talk, and reason immediately after the accident while still sustaining profound damage to the systems underlying executive function and social behavior.

"Gage Was No Longer Gage"

Before the accident, Gage had been described as responsible, capable, energetic, and an effective manager of men. Harlow documented significant changes following the accident: Gage became fitful, irreverent, grossly profane, impatient, and obstinate. He was "capricious and vacillating, unable to settle on any of the plans he devised for future action." His former employers considered him so changed they would not return him to his old position.

Harlow's summary became famous: "Gage was no longer Gage."

The case provided early and compelling evidence that the frontal lobes — the parts of the brain damaged by the tamping iron — were not merely passive tissue but played a specific role in personality, judgment, impulse control, and social behavior. Before Gage, the brain had often been regarded as a relatively undifferentiated organ. After Gage, the concept of cerebral localization — that different regions perform different functions — gained significant traction.

But the historical record requires caution here. The most extreme popular retellings portray Gage as permanently transformed into a violent or uncontrollable man. Harlow documented genuine changes, but surviving records from others who knew Gage personally are limited, and later writers embellished his story considerably. The reality appears to have been more complex.

Chile, Stagecoaches, and Neuroplasticity

After his recovery, Gage briefly became a minor celebrity, reportedly appearing with P.T. Barnum's American Museum and touring New England cities where he displayed himself and his tamping iron. Around 1852, he traveled to Chile, where he worked as a stagecoach driver on the demanding Valparaíso-Santiago route for several years — a job requiring complex navigation, management of horses and passengers, daily routine, and sustained social interaction.

Modern researchers have pointed to this chapter as meaningful evidence of neuroplasticity — the brain's capacity to reorganize and adapt following injury. A man incapable of maintaining social conduct or managing complex tasks, as the extreme version of the Gage myth portrays him, could not have functioned as a long-distance stagecoach driver. Whatever personality changes the accident produced, Gage's brain appears to have adapted to some degree in the years that followed.

He returned to the United States and died on May 21, 1860, probably following a series of epileptic seizures, at the age of 36 or 37. His skull and the tamping iron were donated to Dr. Harlow and eventually to Harvard, where they have been preserved in the Warren Anatomical Museum since 1868.

"An iron flew, a life remained,
A wounded mind somehow sustained.
And through one man's unlikely fate,
We learned the brain can shape our state."

History reminds us: the human brain does not merely run our bodily systems — it anchors our character, our choices, and our identity. Phineas Gage's case didn't prove that cleanly or completely. But it opened a window that medicine had never had before: a documented, examined human life in which a specific physical injury to a specific region of the brain produced specific changes in the person who inhabited it. The window has never closed.
